Authorization to Fundraise Packet

An Authorization to Fundraise packet is required every year from PTO, PTA, Booster, and other parent organizations in order to raise money for Oceanside schools.

The packet includes the following:

  • Authorization to Fundraise form, including organization signature and principal's signature
  • The organization's bylaws, if there are any changes
  • Verification of tax-exempt status
  • Copy of prior year tax return filed with the IRS and CA Franchise Tax Board
  • The organization's officers for the current school year
  • Certificate of Liability Insurance maintained by the organization for the current year
  • Voided bank check, if new bank account was opened
  • List of persons authorized to withdraw funds from bank account
  • Treasurer's report from prior year showing income, expenditures, and ending balance in June

Below is more information regarding each of the required items.

Authorization to Fundraise form

This is the form required in the packet.  Please list all possible fundraising activities, along with their dates.

This form requires a signature from a governing member of the organization, along with the signature of the site principal.

A copy of the form will be returned to organization after approval by the Superintendent or designee.

Organization's Bylaws

Each organization, whether it be a PTO, PTA, or Boosters, should have a set of bylaws.  These bylaws include procedures for running the organization, governing board structure, and policies by which the organization follows.

The bylaws should be kept with all of the organization's materials, in addition to having a copy readily available at the school site for reference.

If there are any changes to the bylaws, please include the change with the Authorization to Fundraise packet.

Verification of Tax Exempt Status

All PTOs, PTAs, and Boosters must be their own entity.  They have their own EIN (Employee Identification Number) through the IRS.  Tax exempt status would be marked on the organization's W9.  Tax exempt status can also be searched for and printed through the IRS website.

Prior Year Tax Returns

A copy of the organization's tax returns must be included in the packet.  This should include the return filed with the IRS and with the California Franchise Tax Board.  A copy should also be kept with the organization's records.

Organization's Officers

Please include a listing of the organization's officers for the current year.  The listing should include name, position on the governing board, email address, and telephone number.  Please also indicate if the officer is an employee of the Oceanside Unified School District.

Certificate of Liability Insurance

Each organization should have liability insurance for the current year.  Oceanside Unified School District should be listed as an additional insured on a separate endorsement.  Coverage should be for at least one million dollars combined single limit liability for bodily injury and property damage, with a minimum two million dollar aggregate for each occurrence and be named as an additional insured on a separate endorsement.  The certificate holder and additional insured must be identified as:

The certificate of insurance and separate endorsement must be submitted with the renewal packet.  Packets will not be approved without the certificate of insurance and separate endorsement attached.

Bank Information

If a new bank account was opened for the organization, a voided check must be submitted with the packet.

Include a list of persons who are authorized to withdraw funds from the bank account.

Treasurer's Report

A complete Treasurer's Report from the prior June must be included in the packet.  The report should include all income, expenditures, and year-ending balance.
